XX大学 毕业论文 慢性活动性乙型肝炎外周血T细胞亚群上活化抗原的表达 2014年6月25日 慢性活动性乙型肝炎外周血T细胞亚群上活化抗原的表达 关键词慢性;乙型肝炎;T淋巴细胞;CD69;HLA-DR 摘要目的探讨慢性活动性乙型肝炎CAHB患者外周血活化抗原CD69、 HLA-DR分子在T细胞表达的变化及其意义。方法 以31例正常人作为对照， 采用免疫荧光三色标记流式细胞术，检测30例CAHB患者外周血T细胞亚群及 其CD69、HLA-DR抗原表达;同时用荧光定量PCR检测患者的病毒DNA载量。 结果CAHB患者外周血CD3 T细胞的百分率63.789.70与正常对照组 比较差异有显著性P0.01, CD4 T细胞的百分率45.888.51明显低于 正常对照组P0.001, CD8 T细胞的百分率44.647.10明显高于正常 对照组P0.001 ;CD3 T细胞CD69的表达6.852.83与正常人对照组 比较差异也有显著性P0.01,其中CD4 T细胞CD69的表达2.310.94 与正常对照组比较差异无显著P0.05, CD8 T细胞表达CD69水平 3.871.81明显高于对照组P0.01 ;CD3 T细胞上HLA-DR的表达 17.094.23与正常对照组之间差异有显著性P0.001,其中CD4 T细 胞中 HLA-DR 细胞7.972.43和 CD8 T 细胞中 HLA-DR 细胞 5.521.48均比正常对照组显著增高P0.01 P0.001 ;CD8 CD69 、 CD8 HLA-DR 细胞的数量与病毒DNA载量之间呈正相关。结论CAHB患 者存在细胞免疫功能紊乱和淋巴细胞异常激活现象,T淋巴细胞的激活以CD8T 细胞为主，其在抗病毒感染中起着重要作用。 关键词慢性;乙型肝炎;T淋巴细胞;CD69;HLA-DR The expression of activated antigens in peripheral blood T lymphocyte subsets in patients with chronic active hepatitis B. AbstractObjective To investigate the expression of special marker for activation of peripheral blood T lymphocytes in patients with chronic hepatitis B virus and its significance. s Immuno fluorescent three-color flow cytometry was used to study the expression of CD69and HLA-DR on T lymphocytes in30patients with chronic active hepatitis B virus and31 normal controls.The viral load was determined by fluorescence quantitative PCR. Results Expression of CD3 T cells were significantly lower in pa-tients 63.789.70 than that in controls P0.01, meanwhile the expression of CD4 T cells was45.888.51, significantly lower than that of the controls P0.001 and the expression of CD8 T cell was44.647.10, significantly higher than that of the controls P0.001 .Detection of CD69on CD3 T cells in patients was6.852.83, significantly higherthan that of the controls P0.01 .NO significant difference in the expression of CD69on CD4 T cells was observed be-tween patients and the controlsP0.05, and the expression of CD69on CD8 T cells was3.871.81, significantly dif-ferent from that of the controls P0.01 .The expression of HLA-DR on CD3 T cells were significance difference between pa-tients 17.094.23 and that in controls P0.001 .The percentages of CD4 T cells expressing HLA-DR in patients was7.972.43, significantly higher than that of the controls P0.01 .Significant difference was observed in the expression of HLA-DR on CD8 T cells in patients 5.521.48 as compared with controls P0.001 .There were positive correlations between percentages of CD8 CD69 , CD8 HLA-DRcells and viral load. Conclusion The findings support that cellular immunodeficiency exists in patients and that T lymphocytes were abnormally activated in the patients.The activation of peripheral blood T lymphocytes in patients mainly involves CD8subset and it may play an important role in the immune response to antiviral infection.
